3. The resulting page is the cover page for your application. Print it out
4. Today's date will appear on your application, so be certain not to download it until you are ready to use it. (Notification that you have begun the application process is sent to the Gladys Brooks Foundation)
Please carefully observe the following or your application will not be accepted:
* No application will be considered if all questions have not been answered.
* No application will be considered unless the cover page is included.
* Please do not alter the space allotments on the application.
* Electronic submissions are not acceptable.
* Please note that the grant period closes June 2, 2008.
** WE WILL NOT ACCEPT GRANT APPLICATIONS AFTER JUNE 2, 2008.
* The application must be completed and mailed with all supporting documents to the foundation within 45 days from the date you download it from this website.
